Objective To investigate the safety and efficacy of balloon-expandable covered stents (BECS) in the treatment of subclavian steal syndrome (SSS).
Methods The clinical data of patients who received BECS implantation for SSS caused by subclavian artery stenosis or occlusion at Xuanwu Hospital, Capital Medical University, from January 2022 to December 2024 were retrospectively collected. The distribution characteristics of clinical data, clinical efficacy, perioperative complications and adverse events were analyzed.
Results A total of 77 patients with left subclavian artery lesions were enrolled. The mean lesion length was (2.4 ± 0.8) cm, and the median lesion diameter was 7.4 (7.2, 7.7) mm. The median preoperative inter-brachial systolic blood pressure difference was 30.0 (26.0, 40.0) mmHg, which decreased to 6.0 (4.0, 7.0) mmHg after surgery (P < 0.001). The median postoperative follow-up time was 26.0 (18.5, 37.0) months. During follow-up, in-stent restenosis occurred in 3 patients (3.9%), while stents remained patent in the remaining 74 patients (96.1%) undergoing stent implantation. An inter-arm systolic blood pressure difference < 10 mmHg was recorded in 75 patients postoperatively. Of the 71 patients with preoperative posterior circulation or upper-extremity ischemia, only 3 remained symptomatic after surgery. The proportion of symptomatic patients decreased significantly compared with baseline (P < 0.001), with a symptom relief rate of 95.8% (68/71). Perioperative complications included peri-femoral artery hematoma in two patients (2.6%) and posterior circulation embolism in one patient (1.3%).
Conclusion BECS are feasible for interventional treatment of SSS. This cohort demonstrated favorable clinical efficacy and overall safety during short-to-medium term follow-up, which can provide a clinical reference for similar cases.
